Please give us your candid appraisal of SSI 2009, your evaluation of SENCER-sponsored materials and programs, and your suggestions for improving the SENCER program. We require your assessment so that we can learn from you and adapt our program to better suit your needs and achieve the objectives the National Science Foundation expects from us. Please take the time necessary to give us your thoughtful responses.
The evaluation has several sections, most are self-explanatory. In addition to the questions that we have posed, we are asked by the NSF to collect and report certain demographic data and your estimates of future impacts. Please help us by filling in all the sections marked by an asterisk (*). If you do not wish to respond to a particular question, you may select "N/A" which translates to "No Response" and is the default selection for many of the questions.
Your responses are confidential. We will not share your responses with anyone outside the SENCER National Office in a way that is personally identifiable. The data provided to the NSF are aggregated. If we should ever desire to use your name in association with something you have written (as in an endorsement or for a follow-up), we will contact you first to ask for your permission.
You can submit the form electronically by clicking the "Submit Evaluation" button at the end of the document. You will receive an e-mail confirmation that we have received your SSI 2009 evaluation. If you do not receive an e-mail confirmation, please contact the SENCER National Office.
We thank you for helping us evaluate the Institute and improving our capacity to improve what we do. Please note how long it takes you to complete the form. You will be able to enter the amount of time it took you to complete the evaluation before submitting the form.
We advise you print a copy of this page for your records before hitting ‘Submit’.”
